 All others will consider Ms. Spinner an “out-of-network provider” and your own plan’s out-of-network benefits will apply.

 In general, it is a good practice to call your insurance company prior to any appointment, and verify your coverage benefits as well as any provider you wish to see.

 Ellen utilizes D.B. Billing & Collections service, in Springfield, OH.  At the time of your appointment at Gentle Care Health Center, a copy of your insurance card will be made, along with your driver’s licence, and any required co-pay listed on your card will be expected.   Please bring a check or cash for any co-pay, as we do not have a credit card machine in the office.  Your insurance and demographics information will be given (confidentially) to D.B. Billing & Collections, who will then bill your insurance carrier.
