Instructions for completing pre-appointment paperwork:

If you are coming to Gentle Care Health Center for an appointment with Ellen Spinner, CNP, you can download and print  the  required documents prior to your appointment.   The documents appear down the left side of this site, and are titled “Health History”, “Patient Information”,  “Consent Form”, and “Privacy Practices“. Please complete all 4 of these forms to the best of your ability, and Ellen or Yvonne will review them with you when you arrive for  your appointment.  If you are younger than 18 years old, and you wish to have your insurance billed for your visit, please be sure to have your parent or guardian sign the consent and privacy forms in the spaces listed.

If your appointment is for a scheduled sports or camp physical, please also print and complete the OHSAA physical forms.  Please be sure to have a parent sign this form in the space provided specifically.

Also, please bring with you your health insurance card, if indeed you have health insurance.  Also bring any office visit co-pay listed on your card (check or cash).  Please also bring your driver’s license, or another photo id to be copied for your chart.

If you do not have health insurance, be prepared to pay some percentage of your visit fee that day (most folks pay at least 50% at the time of service).  You can set up a payment plan for the remaining balance, if desired.
